As a patient receiving physical therapy, occupational therapy, or speech-language pathology services from Gold Coast Physio, you have the following rights:
Right to Dignity and Respectful Care
You have the right to receive care that is considerate, respectful, and dignified — regardless of your age, race, ethnicity, religion, gender, sexual orientation, disability, or source of payment. Our clinicians will address you by your preferred name, honor your cultural and personal values, and treat you as a whole person throughout your care.
Right to Informed Consent
You have the right to receive a clear explanation of your diagnosis, proposed treatment plan, expected benefits, potential risks, and available alternatives — in language you can understand. You have the right to ask questions, receive honest answers, and give or withhold your consent before any treatment begins. You may revoke consent at any time.
Right to Participate in Your Care
You have the right to actively participate in developing and modifying your therapy plan. Your goals, preferences, and priorities will be incorporated into your plan of care. You are encouraged to ask questions, share concerns, and communicate openly with your therapist about your progress and treatment at every visit.
Right to Refuse Treatment
You have the right to refuse any treatment, procedure, or portion of care at any time, without penalty or compromise to your access to future services. Your therapist will explain the potential consequences of refusing care so you can make a fully informed decision.
Right to Privacy and Confidentiality
You have the right to privacy during your therapy sessions and the right to expect that your health information will be kept confidential in accordance with federal HIPAA regulations. Information about your care will be shared only as necessary for treatment, payment, and operations, or as required by law. Please see our Notice of Privacy Practices for full details.
Right to Access Your Health Records
You have the right to inspect and receive a copy of your clinical and billing records in a timely manner. You may also request that corrections or additions be made to your health record if you believe information is inaccurate or incomplete. Please contact our office to submit a records request.
Right to Know About Fees and Financial Responsibilities
You have the right to receive a clear explanation of the cost of your care, your insurance coverage and benefits, and your estimated out-of-pocket responsibility before treatment begins. We will make every effort to verify your insurance eligibility and provide you with estimated costs upfront. You have the right to request an itemized statement of charges.
Right to Know Your Care Team
You have the right to know the names, credentials, and roles of the licensed professionals providing your care. You have the right to request a change in your assigned therapist if you believe a different clinician would better meet your needs, subject to availability.
Right to Non-Discrimination
Gold Coast Physio does not discriminate on the basis of race, color, national origin, age, disability, sex, or any other protected characteristic in the provision of services. If you believe you have experienced discrimination, you have the right to file a complaint.
Right to Voice Concerns and File a Complaint
You have the right to express concerns about your care and to file a complaint without fear of retaliation or negative consequences to your care. Complaints may be submitted verbally or in writing to our Privacy Officer. You also have the right to contact your state health department, Medicare, or other relevant oversight agencies at any time.
Right to Be Informed of Changes
You have the right to be notified in advance of any significant change in your care plan, discharge from services, or any change in the services Gold Coast Physio provides that may affect your care.
Right to Communication Assistance
You have the right to receive information and communicate in a language you understand. We will make reasonable efforts to provide interpreter services or translated materials where needed. You also have the right to receive information in accessible formats if you have a visual, hearing, or other communication impairment.
Patient Responsibilities
To help us provide the best possible care, we ask that you:
- Provide accurate and complete information about your health history, current medications, and condition
- Participate actively in your therapy plan and communicate honestly about your progress
- Notify us promptly if you need to cancel or reschedule appointments
- Meet your financial obligations related to the cost of your care
- Treat all Gold Coast Physio staff with courtesy and respect
